Yellow Card Scheme

If you experience a side effect to a medicine or experience a problem with a medical device you can help make them safer by reporting to the Yellow Card Scheme.

What to Report:

You can report suspected side effects to any medicine or herbal remedy, whether it is prescribed by a Healthcare Professional or bought without a prescription.  You can report side effects that have happened to you personally, your child, or someone you are responsible for.  You should also tell your doctor about the side effect.

How to Report:

You can report suspected side effects in the following ways:

• By using the Online Yellow Card form at www.mhra.gov.uk/YellowCard                                        
• By using the free Yellow Card app from the Apple store or Google Play Store
• Over the telephone by calling 0800 731 6789 (Mon-Fri 9am to 5pm)
