Repeat Prescriptions

IMPORTANT: When ordering your medication, please allow 72 HOURS to process the request.  It is your responsibility as a patient to ensure you have an adequate supply of your medication.

Please email any repeat prescription requests to the surgery you are registered at.

In Person
You can attend the surgery to make a prescription request, or drop off a request in the prescription post box outside the entrances at both surgeries
By Post

Repeat prescriptions can be requested using the white repeat slip which is attached to your previous prescription.  Tick the items required and either post it, fax it or place in the letter box at the practice entrance.  Alternatively use a piece of paper to write your name, address, date of birth and medication needed.

If you have an account, you can request repeat prescriptions using The NHS Wales App

 

REMEMBER:  Pharmacies offer the Common Ailments Service and a Walk-in Centre facility.  They can provide confidential advice and issue FREE prescriptions, including emergency hormonal contraception and emergency supplies of medication.
